Antioxidants and zinc in Age related Macular Degeneration by Michael Lange OD, CNS

I think most Optometrists and ophthalmologists will now agree that nutritional supplements are beneficial for macular degeneration patients. AREDS 1 and AREDS 2 studies have helped us to determine what is good and what is bad for our patients.  There are also many other good nutritional studies that influence the optometrist or ophthalmologist in a specific nutritional regiment. I think most nutritional eye care experts will agree that beta-carotene, synthetic high dosages of vitamin e, high dosage of zinc oxide, F, D and C red, yellow and blue dye are all bad in an eye vitamin.  Many studies are now linking potential beneficial effects of resveratrol, lutein, zeaxanthin, astaxanthin, coq-10, acetyl l carnatine, alpha lipoic acid, B complex, green tea polyphenols, taurine, bilberry, blueberry, vitamin D3, triglyceride form omega 3 fish oil and many more to retinal health.

Genetic testing is now coming into play when making recommendations of specific nutritional supplements for macular degeneration.  A patient can now walk into an eye doctors office and have a specific genetic test done that will help the doctor to determine what specific antioxidants  may or may not be beneficial for their macular degeneration. A very recent analysis of a genetic database of patients in the AREDS study supports the pharmacogenomic selection of nutritional supplements for macular degeneration.  This analysis found that patients with certain specific genotypes  may react differently to zinc.  Some of these patients  react favorably to the intake of zinc along with antioxidants and some react negatively to zinc when combined with antioxidants.  Patients will need to get a genetic test done to help determine whether or not they should be on a zinc free eye vitamin or an eye vitamin with zinc.

Our knowledge of supplementing with zinc has changed dramatically over the years.  Zinc in high dosages can be toxic to the body especially the urinary tract, brain, prostate and the retina.  Eye vitamins without zinc today are hard to find.  Multivitamins without zinc are equally hard to find on todays vitamin shelves.  I personally recommend no one taking higher than 40 mg of zinc in a supplement do to the potential toxic side effects. The new AREDS 2 STUDY INDICATES THAT SUPER HIGH DOSING OF ZINC IS NOT NECESSARY FOR RETINAL HEALTH.
